The band first single of four they released in short career. This one is special because it comes with the band's Fan Club application. 
The application has never been filled out and is in good condition, light wrinkles at the edges and the top edge has a very small tear.

Condition: Vinyl is high VG+, closer to NM- visually.  Both sides look almost perfect. Both sides play almost at NM-, very faint sometimes very minor distorted at top ends but very minor. Labels are NM-, both sides are clean and perfect. Includes Fan Club form, NM- and almost perfect. Sleeve is generic white.
